COVID-19 Vaccinations are available at
New York-Presbyterian Vaccination site
at the Armory (ADA accessible)
(located at 651 West 168th Street). 

For those 18 to 64
with underlying medical conditions
& 65 years or older.

Appointments are required
to be vaccinated and must be scheduled prior to arrival.

YOU WILL NOT RECEIVE A VACCINE WITHOUT AN APPOINTMENT.

The Armory is open for appointment
between the hours of 7:00 AM-9:00 PM.

To obtain an appointment you will need to provide
the following information:
Name, Date of Birth,
Address, and Phone number.

Recommendations Prior to Patent Arrival:

You will be able to bring one visitor,
such as a family member or an aide. 
At this time only the authorized person will be vaccinated.

Masks are required for both you and your visitor to enter. 
If you do not have a mask, one will be provided to you.

Please bring your insurance card to the Armory
on the day of the appointment.

When you arrive on-site, you will have guidance on where to go.
Look out for signage and staff to point you to the patient vaccine clinic.

Key Notice:

Patients with current COVID-19 diagnosis, symptoms or exposure
will NOT be able to be seen at the Armory,
but you will have an opportunity to receive the vaccine in the future. 
Please let us know ahead of time the following.

Did you have acute illness
with a fever of greater than 100 degrees in the past 24 hours?

Have you been diagnosed with COVID-19
in the past 10 days?
